One of my five month old twins spit up a few times today and it was egg yolk yellow. She has a low grade -101.5F Rectal- fever but otherwise seems fine. She is exclusively breastmilk fed and my milk is always white. She has been on Zantac for reflux since she was a newborn, with no changes in meds, etc. Any ideas what could cause the yellow? She does cough occasionally...

It sounds to me like, if she has a fever, she might have a sinus infection. It could be post-nasal drip. All my kiddos cough when their sinuses drain. Alternatively, it could be bile. That might be something your pediatrician would want to hear about, so you might want to give a call

I did call the pediatrician, and he said not to worry unless she was showing signs of respiratory distress. I talked to the nurse the next morning and she said that it could be bile, and watch her for uncontrolled fever or additional bright yellow spit-up/vomiting. She seemed fine the next morning. Thankfully it was nothing serious!
